Our association has always valued the physical and mental well-being of
its members. In conjunction with our planned future activities,
we would like to solicit opinions from all members.
Please click any number of hearts,
5 = Most Interested and 1 = Least Interested.
Due to budget constrain, all of the following activities are subject to a fee.

決定徵求各會員的意見請在此問卷，填寫閣下對以下各種活動作出選擇。
請選擇: 五個心形 = 非常有興趣，一個心形 = 沒有興趣。
因為經費上的考慮，以下所有活動可能收費。

Disclaimer

1. Activities of the Club must be in alignment with the Club’s Purposes as defined in the Rules of the Club 2014. The main ones are to promote the social, educational, intellectual and general welfare of the member.
2. The activities must be self sustainable cost wise.
3. The survey responses would help us to plan but it may not be possible to run some of them due to limited resources.
